Job Description:
Provide OT advice and assistance to all departments within Caerphilly CBC, relevant partners, third sector organisations, health professional on matters relating to both occupational therapy and housing for disabled persons.
Assisting the Private and Public Sector Housing service together with other social landlords to maximise the number and optimise the use of accessible or adapted homes throughout the County Borough.
Provide specialist support relating to adaptations to meet specific needs of individual households where improvement works are to be undertaken to meet the requirements of the Post Asset Management scheme.
Address the objective identified by Welsh Government to maximise the use of accessible or adapted homes.
Housing Strategy and Development are supported through the identification of the demand for accessible housing and the geographical areas of that demand.
Undertake OT assessments of Housing applicants referred via the Common Housing Register whereby a medical need is identified.
Provision of OT advice and support to all teams within Caerphilly Homes and to registered social landlord partners where appropriate.
The assessment of need of residents and preparation of professional recommendations to address those needs, undertaking consultations with tenants, carers, health and social care professionals, officers, contractors, consultants, and Elected Members as and when appropriate.
Assist in the efficient running of Public Sector Housing contracts, such as PAMS, to avoid delays, ensure tenants with special needs are supported in an appropriate manner, and quality standards are maintained.
Responsible for overseeing the identification, categorisation and recording of accessible housing.
Assess Common Housing Register applicants with physical health conditions or disability to determine their accessible housing requirements, taking into account prognosis, long term need and family commitments as well as support needs. Advise the Common Housing Register team accordingly.
Assess applicants to determine whether granting of reasonable or additional preference is appropriate for their housing application.
Provide advice and support to applicants and tenants in relation to the suitability of property they are offered or which they currently occupy.
Develop and maintain a detailed working knowledge of the Common Allocations Policy, Common Housing Register and Accessible Housing Register.
Identify, categorise, and record accessible housing and best match disabled applicants with suitable accommodation based on their specific requirements.
Provide specialist impartial OT advice in cases presented to the Housing Assessment Panel and Housing Review Panel.
Assess for adaptations required to meet the specific needs of either a prospective social housing tenant or current occupier and make suitable recommendations for any necessary works (minor or major).
Assist in facilitating complex hospital discharge cases where a significant or complex housing need has been identified.
Support Housing Strategy and their partners, by identifying specific needs of prospective occupiers in relation to both new build projects and conversion of existing properties.
Work with architects and contract managers in the design and development of new build projects or adaptation of existing development quality requirements properties for specific occupiers.
