Main duties of the job
You Will Be Required To:
* Autonomously deliver a caseload of patients with highly complex needs, using evidence-based, client centred principles to critically analyse, assess, plan, implement and evaluate interventions. This will include education, health promotion and support on all aspects of the patient's treatment and management plan.
* Work collaboratively with other members of the multi-disciplinary team across different pathways and care settings to ensure the health care needs of patients are met. This will include continual evaluation of the current service and reflection on own practice.
* Act as an expert resource to other health professionals across a number of settings, monitor and support services, and participating in pathway development, education research and audit, whilst continually developing and updating knowledge and highly specialist skills required for the role.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
