We are seeking an enthusiastic, self-motivated and well organised First Contact Physiotherapist to work alongside and support our multi-disciplinary team within our Primary Care Network. You will be a dedicated professional who shares our values and passion for delivering high quality, joined-up clinical management of musculoskeletal conditions.
Main duties of the job
You will have the clinical expertise andability to autonomously assess, diagnose and treat patients with a range ofconditions and be comfortable triaging and seeing patients without priorcontact with their GP. You will have experience in managing caseloads and beconfident in formulating rapid and accurate diagnosis supported bycomplementary management plans.

Job responsibilities
1. Key Duties & Responsibilities:
Work independently, as an extended scope practitioner, to assess, diagnose, triage, and manage patients, taking responsibility for the management of complex caseloads.
Work with patients who self-refer or from a clinical professional within the PCN
Undertake advanced assessment of musculoskeletal patients as a first point of contact, diagnosing and giving advice on management and referring onto other services as appropriate.
Arrange clinical investigations, including X-Rays, pathology tests, and others as indicated to ensure accurate diagnosis and safe and effective condition management.
Provide clinical expertise and best practice to assess, plan, implement and evaluate interventions to ensure patients receive timely access to care.
Work as part of a multi-disciplinary team in a patient facing role, using expert knowledge of movement and function issues, to create stronger links for wider services through clinical leadership, teaching and evaluation;
Develop integrated and tailored care programmes in partnership with patients, providing a range of first line treatment options including self-management, referral to rehabilitation focussed services and social prescribing;
Develop and make use of full scope of practice, including skills relating to independent prescribing, injection therapy and investigation.
Deliver programmes of supported patient self-management, in ways that facilitate behavioural change, optimise individuals' physical activity, mobility, fulfilment of personal goals and independence, optimising patient outcomes.
Provide leadership and support on MSK clinical and service development across the PCN, alongside learning opportunities for the whole multi-disciplinary team within primary care;
Develop relationships and a collaborative working approach across the PCN, supporting the integration of pathways in primary care;
Encourage collaborative working across the wider health economy and be a key contributor to supporting the development of physiotherapy clinical services across the PCN.
Liaise with secondary and community MSK services where required, using local social and community interventions as required to support the management of patients within the PCN
Critically evaluate current research and demonstrate an understanding of the application of national and local guidelines and legislation relating to health and social care and their impact on service provision.
Be responsible for and lead others in maintaining stock of equipment and materials, advising the correct procedures relating to the safe and competent use, issue and retrieval.
Actively participate in the mentoring of colleagues and participate in the on-going development of a physiotherapy service within the PCN.
Implement all aspects of effective clinical governance for own practice, including regular audit and evaluation, supervision and training.
Maintain a professional portfolio for CPD recording learning outcomes through participation in internal and external development opportunities and to demonstrate acquired highly specialist skills and knowledge of professional practice.
The successful candidate will be an experienced HCPC registered First Contact Physiotherapist.
Duties will vary from time to time under the direction of the clinical director and network management leads, in agreement with the post holder, dependent on current and evolving practice workload and staffing levels.

Qualifications

9. BSc Degree in Physiotherapy
10. Masters Level qualification or the equivalent specialist knowledge, skills and experience
11. A minimum of one years post qualification experience.
12. HCPC Registration
13. Be a member of CSP or appropriate professional body
14. CPD pertaining to Primary Care and MSK management
15. Can demonstrate 5 years of working at Level 7 capability in MSK related areas of practice or equivalent (such as advanced assessment diagnosis and treatment)
16. Can demonstrate ability to operate at an advanced level of practice
17. Holds the relevant public liability insurance
