Job Summary
A new and exciting opportunity for a skilled part‑time Health Care Support Worker (HCSW) to contribute to the development of a redesigned Nursing Care Model with the Hillingdon Care Connection teams (CCT).
The CCT comprises a Guided Care Matron, GP, HCSWs, an administrator and well‑being and support assistants, working together to provide active case management for adult patients with long‑term conditions to prevent unnecessary hospital admissions.
The successful applicant may have contact with patients or service users and is expected to drive and have access to a car as community staff travel considerable distances daily.
Responsibilities
* Home visits for clinical tasks
* Joint visits with matrons when required
* Observations and NEWS2
* Occasional medication monitoring – one week maximum
* Phlebotomy
* Simple wound care
* Eye drops – short term
* Supportive reviews
* Urinalysis
* Inhaler technique
* Pressure area checking
* Documentation
Following a robust induction you will be expected to work independently, ensuring any concerns are escalated in a timely manner to the Matron or GP who have overall clinical responsibility.
HCSW role and expectations
Band 3 HCSWs work closely with people who use our services, their families, friends and carers, contributing to how people experience our services. They are expected to be kind, responsive, professional and informative, and contribute to the quality of services by:
* Keeping the people who use our services informed and working closely with Registered Nurses
* Contributing to the best possible clinical outcomes by using up-to-date skills and adhering to evidence‑based policies and procedures when implementing care plans
* Ensuring the people using our services have a good experience by respecting, empowering and working in partnership with them when implementing and evaluating care plans
We believe that the best healthcare is delivered by multidisciplinary teams working well together and in partnership with other teams and services to provide seamless care. Band 3 HCSWs contribute to the team achieving its objectives by working closely with and drawing support from Registered Nurses and other health professionals.
