We are seeking an experienced Hospital Social Worker to join our Hospital Discharge Team in Hull. The role involves working collaboratively with hospital staff and community partners to facilitate safe and timely discharges, ensuring service users receive appropriate support and care planning.
Key Responsibilities:
Undertake assessments within hospital settings.
Support effective discharge planning and multi-disciplinary collaboration.
Liaise with health and social care professionals to ensure continuity of care.
Maintain accurate case records and documentation in line with local authority standards.
Requirements:
Qualified Social Worker (QSW) with current Social Work England registration.
Proven experience in hospital discharge or hospital-based adult social work.
Ability to travel between Hull Royal Infirmary and Castle Hill Hospital.
Excellent communication and organisational skills.
