Practice Nurse - Hope Family Medical Centre
Hope Family Medical Centre is a friendly, happy place to work with 7 GPs, 2 APPs, 4 Practice nurses, 2 Health care assistants and a paramedic practitioner. We support a practice list of 10200 patients in a large semi-rural setting.
The postholder will be expected to work at Hope Family Medical Centre to provide core primary care nursing and chronic disease management. This will involve working with the Practice nursing team and GPs to provide clinically effective care and support the practice in achieving quality indicators and targets.
Main duties of the job
We are recruiting for a fully trained Practice Nurse.
The Practice Nurse will be required to hold patient consultations within the Surgery, carrying out chronic disease management of Diabetes, Asthma, Heart disease (and providing healthy living advice) and routine screening e.g smears, immunisations (adults and children) and wound management. Compliance with Nursing Code of Conduct will be expected.
It would be desirable to have experience in contraception and travel vaccinations but not necessary, as training can be provided.
This role is part time, we are looking for a Practice Nurse to work 2 days a week.

We practice from a new Health Centre in a pleasant large village between Mold and Wrexham, 10 miles from Chester and commuting distance to Liverpool and Manchester. Our list of patients is varied, from farmers to factory workers, commuters to stately homeowners.
Job responsibilities
Competence in administering vaccinations, including routineadult and seasonal immunisations. Experience delivering childhood immunisationsin line with national vaccination schedules. Proficiency in cervical screening,including managing nonresponder recalls and ensuring timely followup.Experience in the management of longterm conditions as part of ongoing patientcare. Ability to undertake hypertension reviews and support patients throughmonitoring and lifestyle guidance. Experience providing anticoagulationservices, including the monitoring and management of patients on Warfarin andDOACs.
Writing records and keeping patients notes up to date isessential and accurate record keeping in line with practice policy and NMCGuidelines. Adherence to practice policies and guidelines on body fluid, sharpsinjury, clinical waste, and manual handling will be required. In addition tobeing up to date with safeguarding training.
The workload will be a mix of planned review appointmentsand some on the day minor illness appointments. The ability to prioritise anduse time effectively in the daily workload and organise patient care isrequired.
The Practice Nurse may be required to provide first aid andmanagement of emergencies alongside the medical team if required.
