We are looking for a qualified General Practitioner (GP) to work part time as the named GP to cover inpatients at Helston Community Hospital. Patients are predominantly admitted for rehabilitation and reablement following an acute hospital stay, or for end of life care. There is provision of a nurse-led CHAD service on the ward.
You will review each patient at least weekly and form part of a multidisciplinary team, working with the patient to enable discharge to their own place of residence as promptly as possible. You will be supported by a resident doctor and an experienced nursing team providing nurse-led care.
There are opportunities to be involved in service development or the opportunity to develop medical leadership through other parts of the Trust (eg learning from deaths, medical student training, quality improvement projects).
The role can be sessional/part time and hours can be flexible to suit the candidate2s work/life balance.
Responsibilities
* In liaison with the senior nurses on the ward, provide senior medical decision-making to inpatients on community hospital ward.
* Provide medical input into the MDT management of patients.
* Support patient flow and alternatives to admission in keeping with the \'hospital to home\' NHS plan.
* Contribute to advanced care planning (eg TEP) and end of life care.
* Supervise and contribute to teaching and training of junior doctors (including GP trainees), medical students, ANPs and other AHPs.
* Liaise with patients, families and carers regarding their medical care.
* Liaise with other medical specialties e.g. Eldercare, Palliative care for specialist advice where needed.
* Contribute to governance, incidents, learning, complaints and other processes (eg legal) for the patients you have managed. Support the Clinical Director in the same.
