The SPA will act as the central hub for coordinating clinical referrals, triaging urgent needs, providing advice and directing patients to the right services at the right time. Whether you're answering a call, reviewing referrals, or coordinating rapid support, you will play a key role in ensuring patients and professionals receive timely, efficient, and compassionate responses.

We’re looking for motivated, compassionate, and solution-focused individuals with excellent communication skills and a passion for integrated care. You’ll thrive in a fast-paced environment and be confident in handling referrals, liaising with clinical teams, and putting patients at the centre of everything you do.
Work in accordance with their professional code of conduct. Be aware of and adhere to BCHC policy and procedures. Ensure evidence-based care is practiced and always promoted using triage tools safely by exercising clinical judgement and expertise. Act as a first contact, exercising a high degree of professional autonomy and critical judgement in providing complex clinical triage assessment for patients referred with acute and long-term condition needs through a range of consultation mediums. Provide advanced assessment skills to recognise red flags and escalate promptly balancing patient safety, service guidelines and patient preference. With support from Advanced Clinical Practitioners and Clinical Practitioners initiate investigations, interpret diagnostic results, plan, manage and evaluate care where appropriate/necessary. Make direct referrals as appropriate for medical assessment and diagnostic procedures within agreed pathways. Signpost, refer and support access to health services to reduce inappropriate attendances and admissions to A&E. Actively promote participation by the team in current quality initiatives (for example, falls prevention). Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately. Promote health by collaborative working with patients and their relatives. To provide clinical expertise within the Single Point of Access to reduce unnecessary hospital admissions and contribute to effective admission avoidance and facilitate early hospital discharge, including history taking, clinical triage, and implementation of a range of care services for patients with acute and long-term conditions. The post holder will work in partnership with patients, carers and other health and social care agencies to promote independence and develop individual plans of care relevant to patient need. The post holder will be expected to work multi-professionally, support other team members’ roles and contribute to the future development of the service by providing professional advice and appropriate training to others in the multi-disciplinary service. The post holder will be expected to rotate into clinical teams periodically to remain up to date with the changing landscape of community services and maintain face-to-face clinical practice. The post holder will be expected to work within a flexible pattern over a 7-day week.
