Social Worker – Hampshire | Family Help Hub | Locations across Hampshire
Role Responsibilities
* Conduct comprehensive assessments of connected carers using professional expertise, within legal and statutory frameworks.
* Coordinate and deliver appropriate care services aligned with departmental priorities, budget constraints, and best value principles.
* Collaborate with partner agencies to safeguard vulnerable individuals in line with statutory duties.
* Manage a caseload effectively, supporting individuals to achieve their goals and build on their strengths.
* Regularly monitor and review care plans to ensure services remain effective and responsive to changing needs.
* Maintain accurate and accessible case records using both paper and digital systems, in compliance with departmental policies.
* Prepare clear, analytical reports to support decision-making and case management.
* Provide up-to-date advice on benefits, fees, and allowances to promote independence and manage expectations.
Requirements
* SWE Registration
* Eligible to work in the UK
* Drive and have access to a car
* Post-qualified experience
* Degree level or equivalent in Social Work
* 3 years permanent experience within an English Local Authority
