Position Overview
This is a full-time remote position, primarily based in Central MA, CT, or Western NY, with occasional onsite meetings or training. The role involves remote patient monitoring and care coordination for patients with chronic conditions.
Salary Range
$60,000 - $62,000 USD per year, based on experience and location.

Key Responsibilities
* Review patient information to determine eligibility for remote monitoring.
* Educate patients and caregivers on program protocols and device usage.
* Assist with account setup in the Brook Health app.
* Ensure accurate documentation and data review.
* Coordinate with the healthcare team to ensure quality care.
* Maintain compliance with HIPAA and other security standards.
* Develop understanding of patient needs and advocate for their health goals.
Qualifications
* Active Practical/Vocational Nursing license in NY, MA, or CT.
* Experience in primary care preferred.
* Proficiency with EMR/EHR systems and healthcare technology.
* Strong organizational and communication skills.
* Ability to work independently and collaboratively.
Note: Candidates must reside within commuting distance of the designated locations. This role is not eligible for relocation or visa sponsorship.
