Job Summary
This role offers a great opportunity to support safe prescribing and medicines optimisation within a general practice setting.
The postholder will focus primarily on high-risk medication monitoring and medicines reconciliation to ensure patients receive safe, effective care during and after transitions between care settings.
Main duties of the job
High-Risk Medication Monitoring:
Identify and monitor patients prescribed high-risk medications (e.g., methotrexate, lithium, warfarin, DOACs).
Ensure required blood tests and reviews are completed in a timely manner.
Collaborate with patients, clinicians, and laboratories to track and act on results.
Maintain accurate clinical records using SystmOne.
Medicines Reconciliation:
Conduct medicines reconciliation following hospital discharge and other care transitions.
Ensure medication lists are accurately updated and reviewed by the clinical team.
Communicate effectively with external healthcare providers and community pharmacies.
Administrative And Clinical Support:
Assist with audits and reviews of high-risk medicines and reconciliation processes.
Support the pharmacy team in promoting safe prescribing and medicines use.
Help respond to medication-related queries from the clinical team.

OUTLINE OF POST:
Diamond Medical Group is seeking a proactive and detail-oriented Pharmacy Technician to join our team.
This role offers a great opportunity to support safe prescribing and medicines optimisation within a general practice setting.
The postholder will focus primarily on high-risk medication monitoring and medicines reconciliation to ensure patients receive safe, effective care during and after transitions between care settings.
MAIN TASKS:
Primary Duties and Areas of Responsibility
Knowledge, Skills And Experience Required
Experience and confidence using SystmOne clinical system.
Understanding of high-risk medication and monitoring requirements.
Excellent attention to detail and organisational skills.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within a multidisciplinary team.
Previous experience working in general practice or a primary care network (PCN).
Familiarity with local Leeds prescribing pathways and processes.
Interest in quality improvement and audit.
Education, Training And Development:
Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
Special Working Conditions
The post-holder is required to travel independently between partner sites (to attend meetings etc. hosted by other agencies.
