Lincolnshire Community Health Service (LCHS) has an opportunity for Registered Nurses to join their Skegness and Mablethorpe Community Nursing Team.
We have vacancies for 1 full-time position.
Overview
As a community staff nurse you will work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, planning, delivering and evaluating nursing care using evidence‑based practice.
Main duties
* Wound management, continence care, palliative care, medication administration and long‑term condition management.
* Co‑ordinate care with the MDT (Social Workers, GPs, Allied Health Professionals) to ensure effective patient care.
* Travel to patients’ homes and clinic settings; transport is essential.
* Work within variable shift patterns: Early 8:00‑16:00, Mid 9:00‑17:00, Late 10:30‑18:30, 7 days a week.
Job responsibilities
The service offers a 12‑week induction programme, robust preceptorship and clinical skills programme with an experienced clinical education team.
Qualifications
* Current UK NMC adult nurse registration (Level 1).
* Evidence of continuous professional development.
* Demonstrated mentorship, supervision or assessor qualification and application in practice.
* Relevant training in palliative care, wound care, elderly care, diabetes or other related areas.
* Completed leadership modules/qualification.
* V100 or V150 Community Practitioner prescribing course.
* Post‑registration qualifications (palliative care, care of the elderly, wound care, continence or other relevant post‑refraction qualifications).
Experience
* Assessment, care planning and delivery.
* Delegation and supervision of others.
* Application of research and evidence‑based care.
* Teaching, disseminating knowledge and skills.
* Team working.
* Health promotion and coaching strategies.
Skills
* Range of clinical skills pertinent to the area of practice.
* Excellent communication skills including record keeping.
* IT and electronic patient record systems proficiency.
* Ability to undertake practice audit.

Salary
£32,073 to £39,043 per annum, pro rata.
