Rate:£21.52/hr PAYE |Hybrid: 2 office days + home visits
Contract:4 months initial |DBS Enhanced|BSL Level 2 Essential
Make a Real Difference
Join our Sensory Team to empower Deaf, Hard of Hearing, and dually sensory-impaired adults in Hammersmith & Fulham. Conduct specialist assessments, craft person-centred support plans, and promote independence under the Care Act 2014.
Key Responsibilities
* Carry out initial assessments & reviews for adults with sight/hearing loss.
* Design strength-based care plans, including direct payment options.
* Provide/demonstrate assistive equipment & collaborate with audiology teams.
* Advocate for service users & raise sensory impairment awareness borough-wide.
* Maintain Mosaic records & liaise with multi-agency partners.
What We're Looking For
* BSL Level 2(essential).
* Care Act 2014 & strengths-based practice knowledge.
* NVQ Level 3/A-Level equivalent; social care/sensory impairment experience.
* Excellent communication, IT skills (Mosaic training provided), & problem-solving.
* Self-motivated, resilient, with advocacy experience.
