The Oaks is a brand new, high-quality supported living service based in Calow, Chesterfield. We are dedicated to providing personalised, person-centered support for young adults with learning disabilities, autism, and complex needs. Our goal is to empower individuals to live independently, safely, and with dignity and to ease the transition from children's care to adult support– while being fully engaged in their local community.
The Role
We are currently seeking Support Workers and Team Leaders (permanent or relief) to join our brand-new team. This is a rewarding opportunity for individuals who are enthusiastic about delivering outstanding care and support in a professional and structured environment. This is an opportunity to get involved from the beginning and build a service that is value driven.
Key Responsibilities
· Deliver high-quality, person-centered support to individuals in line with their care and support plans.
· Promote independence and support individuals with daily living tasks, including personal care, meal preparation, medication administration, and community activities.
· Build positive, professional relationships with service users, families, and external stakeholders.
· Maintain accurate records and documentation in compliance with regulatory standards.
· Contribute to a safe, supportive, and inclusive environment for all service users.
About You
· Experience in health and social care is desirable but not essential – full training is provided.
. Team Leader role requires a Level 3 Diploma in adult care or equivalent or willingness to work towards it.
· A compassionate and respectful approach to care and support.
· Excellent communication and people skills.
· Flexibility to work a range of shifts, including weekends and bank holidays.
· A genuine commitment to promoting independence, dignity, and choice.
