Band 4 Assistant Practitioner – Community Stroke Team
An exciting opportunity has arisen within the Clyde Community Stroke Team for an experienced AHP Assistant Practitioner to provide ongoing rehabilitation for patients at home.
The team facilitates timely discharge from hospital for patients following a stroke, and provides continued rehabilitation across the stroke recovery pathway at home
The ideal candidate will be an experienced, motivated and enthusiastic AHP Support Worker who has experience of working within a rehabilitation setting.
The post holder will carry out specific delegated tasks to support AHP and Nursing staff in providing this service.
Patient documentation and multi-disciplinary communication within the team is also a requirement of the role.
Regular supervision and support will be offered as well as training opportunities as required.
