An exciting opportunity has arisen for Nurses / Social Workers / Allied Health Professionals to join the community CAMHS Service working as part of a multi-disciplinary team in a great location. The team is a very supportive, friendly and welcoming well-established multi-disciplinary team. We are looking for health or social care practitioners with knowledge and experience in mental health, working with children and families with moderate to severe complex needs and risk. This role will offer support for mental health and behavioural challenges presentations of or awaiting assessments for neurodevelopmental conditions, working alongside specialist clinicians, supporting Children & Young People (CYP) and families in line with the iTRHIVE framework. The postholder will establish strong links with education and social care throughout the range of interventions from early help to child protection. The post holder will have the responsibility for the provision of a responsive community outreach service for those children who need an integrated and flexible approach ensuring engagement and a needs led service. Due to the nature of this role, the post holder would be required to travel throughout the community, as well as working within the clinic. This role will also support young people transitioning to adult mental health services where required. To participate in a holistic mental health assessment of children and young people referred to the team to make a professional assessment and identify mental health problems early in their development. To promote the emotional and mental health needs of children and young people, organise and disseminate complex facts and situations, interpret information and give informed judgments on possible options to care. To promote early identification and intervention for those children and young people in line with the CAMHS Service model and iTHRIVE framework. To be active in implementing a multi-agency working relationships that ensures integration of care. To plan and collaborate with other professionals and agencies within the locality in the development of a range of evidence-based strategies and interventions to promote the psychological and emotional health of children and young people. Undertake case management and provide evidence-based interventions for those assessed to have mental health and neurodevelopmental concerns. In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.


Responsibilities

* Undertake case management supporting evidence-based interventions for CYP and families within the practitioner's caseload, from the clinic or in the community.
* To work across agencies and boundaries to develop a coordinated response to children's mental health and/or neurodevelopmental conditions.
* To work within the CAMHS systems to promote early intervention, prevention and promote the development of positive emotional and mental health.
* To conduct comprehensive risk assessments of CYP and families, formulating risk management plans including liaison with appropriate agencies regarding risk issues.
* To plan, formulate and provide individual packages of care and psychoeducation for children/young people and their families/carers that takes into account the complexities of family systems, and their professional and significant social networks where there may be resistance to acceptance of outcomes and recommendations.
* To contribute a professional perspective, practices and safeguarding expertise to care planning alongside the multi-disciplinary team and wider clinical team.
* To provide a consultation function to other professionals for children and young people referred into the service to ensure their emotional and mental health needs are prioritised following assessment.
* To build professional relationships with children and young people, their families/carers and to use that relationship to promote their mental health and safeguarding needs.
* To interpret complex and sensitive health information and reach conclusions based on clinical judgement, research evidence and management of risk in order to promote and improve individual health outcomes.
