Camden is seeking a dedicated and experienced Social Worker to join their Mental Health Team. The successful candidate will play a crucial role in supporting discharges from inpatient mental health settings and contributing the Social Work voice within our integrated service. This role involves working collaboratively with a wide range of professionals and agencies to support individuals who have been admitted or detained under the Mental Health Act. Key Responsibilities:
1. Discharge Support: Facilitate and support discharges from inpatient mental health settings, ensuring smooth transitions and continuity of care.
2. Integrated Service Contribution: Provide the Social Work perspective within an integrated service, collaborating closely with multidisciplinary teams.
3. Case Management: Support the management of the social work service for adults with mental health needs, working alongside the Team Manager and Service Manager.
4. Camden's Model of Social Work: Embed and promote Camden's Model of Social Work and strengths-based practice throughout the service.
5. Care Act Assessments: Conduct Care Act assessments and reviews for individuals with psychosis and complex needs.
6. Legislation and Policy Compliance: Maintain a strong knowledge of and adhere to relevant legislation, policies, and procedures, including Safeguarding, Mental Health Act, Care Act, and Mental Capacity Act.
7. Team Collaboration: Work effectively as part of a multidisciplinary team, engaging with professionals from various fields to provide comprehensive care.
8. Interpersonal Skills: Utilise excellent interpersonal skills to engage with service users from diverse backgrounds and ensure their needs are met.
Qualifications and Experience:
9. Qualified to degree level or equivalent with a Social Work England registration
10. Experience working with adults with psychosis and complex needs.
11. Experience conducting Care Act assessments and reviews.
