We are currently seeking highly motivated Community Staff Nurses to join our well-established District Nursing Team at The Mid Yorkshire Teaching NHS Trust. This is an excellent opportunity for both newly qualified and experienced nurses who are passionate about delivering safe, high-quality care in the community setting.

Our District Nursing team operates across six networks, delivering a broad range of skilled nursing care to patients primarily in their own homes. The team consists of qualified District Nurses, Senior Nurses, Staff Nurses, and Clinical Support Workers, all working autonomously to meet complex healthcare needs.

Working as part of our team, you will:

* Gain experience across a variety of community clinical settings.
* Develop advanced clinical assessment and case management skills.
* Be involved in key areas such as hospital avoidance, long-term condition management, wound care, and end-of-life care.
* Plan and deliver complex, person-centred care plans.

Main duties

The Registered Nurse will contribute to a nursing culture that embraces change, fosters innovation, and remains open to new ideas and approaches to care.

* Participating in Clinical Supervision to support reflective practice and continuous improvement.

Person Specification: Registered General Nurse (Community)

Criteria Essential Requirements

Qualifications

* Registered General Nurse
* NMC Professional Registration
* 1st Degree or willingness to work towards
* Relevant post-registration qualification
* Evidence of recent clinical updates

Experience

* Effective communication skills (verbal and written)
* Understanding of early discharge and admission avoidance, including interdisciplinary care needs and implications for Intermediate Care
* Teaching and educational skills to support the learning of others
* Ability and willingness to work independently
* Commitment to personal development and expanding clinical skills

Knowledge and Awareness

* Awareness of the evolving NHS agenda and service priorities
* Awareness of personal limitations and willingness to seek support when needed

Skills and Abilities

* Ability to assess, plan, implement, and evaluate nursing care plans
* Excellent written and verbal communication skills
* Ability to interpret and apply clinical guidelines and policies
* Experience or understanding of multi-agency and interprofessional working
* Strong organisational and time management skills
* Basic IT literacy and ability to use electronic health records

Personal Attributes

* Physically able to undertake manual handling duties and work effectively in a clinical environment
* Other Requirements - Ability to travel between various community sites across the district
* Access to and use of own vehicle
* Full UK driving licence
* Willingness and availability to work flexible shifts, including weekends, unsocial hours, and bank holidays
