Job description This post specifically involves monitoring, risk managing and supporting service users with complex needs who have been convicted of a variety of offences and who have been assessed within statutory public protection frameworks (PPANI) as being of medium to high risk of reoffending. To undertake specific support activities/interventions for service users as allocated by the social worker. This will include the following: Housing and resettlement. Health, Physical and Mental health, substance misuse. Independent living skills. Education and Training. Family and Social Support. Finances and Benefits. Social activities and hobbies. In conjunction, with the social worker, monitor outcomes and adjust interventions as directed. You will be based within Externs Approved Premises and will work in partnership with the services social workers to provide support to services users in order to achieve positive outcomes in resettlement to appropriate accommodation in the community. Please see employer website for full job details & person specification. Responsibilities Work to a residential rota which includes day, evening, overnight and weekend shift work. Ensure a flexible approach to service delivery which will require working across both hostels depending on need and risk. Develop positive working relationships with service users to support community reintegration. Work in partnership with Social Workers to support and risk manage service users who have complex needs including an offending background. Contribute to the assessment of service user needs for referrals to external agencies and support service users to access appropriate services e.g. registration with Doctor, mental health services, Extern Works. Assist in the case management of identified service users and engage in a co-working partnership with Social Workers to meet and manage service user need and risk. Facilitate and participate in group and individual work with service users to deliver on specific interventions to support the achievement outcomes in the areas agreed with the social worker. Promote and encourage independence and assist service users to develop life skills e.g. cooking, cleaning, budgeting and personal care. Support service users to attend meetings/appointments, when appropriate, in order to advocate and support them. Support service users to complete paperwork/online documentation when required e.g. benefits, housing etc. Contribute to the overall delivery of residential services including supporting residents to prepare meals, undertake cleaning of communal areas and complete low level maintenance tasks in the vicinity of the hostel. Assist Social Workers in the development of service users risk assessments and contribute to the continuous review and amendment of same as and when required. Follow necessary risk management requirements e.g. lone working, on call procedures. Ensure completion of hostel risk management procedures e.g. health and safety, fire checks, room checks, medication supervision. Work with service users who exhibit challenging behaviours in relation offending, social skills, substance misuse, learning disability and physical and mental health. Ensure safe practice and environment in the management of service users who present with challenging and complex behaviours. Multi-agency and multi-disciplinary working. Attend and participate in internal and external meetings as required and act as advocate for service user when appropriate e.g. team meetings. Develop and maintain appropriate professional relationships with service users, colleagues and stakeholders. When required, liaise with appropriate relatives. Ensure compliance with necessary recording requirements and responsibility for accurate and timely completion e.g. Eclipse, service user files, case notes. In conjunction with senior staff, assist in the informal supervision and training of new staff, students and relief staff. Prepare for and participate in supervision in line with NISCC and agency guidelines. Adhere to safeguarding procedures and relevant internal and external policies and standards at all times. Report safeguarding concerns, in a timely manner, in line with Extern policy. Attend mandatory safeguarding training. Responsible for keeping abreast of current legislation, particularly within the Criminal Justice System, and Externs policies and procedures, and attend appropriate training as and when required. Adhere to Externs Health & Safety Policies and Procedures e.g. Lone Working Policy, Guardian 24. Skills and Qualifications Essential Skills Minimum of a Level 2 qualification (as set out in the RQF Framework) in a relevant field/subject. Or In the absence of the relevant qualifications 1 year's experience of working within social care dealing with challenging behaviours. 6 months experience of working within social care dealing with challenging behaviours. Ability to demonstrate in application form/at interview, communication skills verbal/non-verbal. IT competent including use of MS Word, Excel and PowerPoint. Be registered or willing to register with NISCC within 3 months of commencement of employment (you will be required to provide evidence of NISCC registration). Desirable Skills Training in the administering of Naloxone. Further Information Salary: £25,559 - £26,071 per annum, pro Time: 1 x 30 hours and 1 x 37.5 hours per week, rota shift working hours, to include evenings, weekends and sleeping nights (flexibility required).
