The Pennine Care Community Rehabilitation Team is looking to recruit a motivated and enthusiastic Senior Support Worker to join our supportive multidisciplinary team. The successful candidate will have experience and knowledge of working with people with chronic relapsing mental ill health, including experience of working across a range of mental health services.

The team supports service users to transition from inpatient mental health rehabilitation facilities to living in the community, supporting people to improve their quality of life and work towards achieving their goals and outcomes. A further aim of the team is to reduce the length of inpatient stay for service users from both acute and rehabilitation wards, incorporating least restrictive practice.

The team has a strong emphasis on the Recovery Model and Social Inclusion, working with service users using a strengths based and trauma informed approach to maximise their independence and autonomy to improve their quality of life.

You will work with a team of experienced practitioners with a strong biopsychosocial, evidence based interventions.

You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.

The team is a trust wide service covering all five boroughs of Pennine Care and it is therefore essential you drive and have access to a car. We are based at Trust HQ in Ashton Under Lyne. Our core hours are Monday to Friday 9am-5pm.


Main duties of the job

* To develop and maintain professional and therapeutic relationships with service users, carers and colleagues in a way that respects their views, autonomy and culture.
* To contribute to collaborative care planning and review processes for individual service users.
* To support service users to identify their own hopes and goals to improve their quality of life.
* To build strong links with the VCSE sector and our partner agencies to collaboratively support the needs of our service users.
* Promote social inclusion, equality and community participation,
* To provide information and support to enable service users to engage in meeting education, employment and vocational needs.
* To undertake therapeutic activity with service users that promotes recovery, is valued and increases independence.
* To assist service users to develop skills for everyday living i.e. budgeting, shopping, cooking, establishing and improving social networks, accessing local services etc
* To engage with families and carers as appropriate as part of service users recovery journey
* To attend and participate in team meetings and contribute to service development.
* To attend and contribute to clinical team meetings, reflective practice and case formulation sessions relating to service users.
* To provide clinical supervision, guidance and mentorship to community support workers in the team.
* Take a lead role in designated areas such as health promotion, physical health initiatives, wellbeing groups or carer engagement.

Job responsibilities

* NVQ Level 4 or BTEC Higher National Certificate or equivalent experience
* Experience of working with people with mental illness and substance misuse
* Experience of community working and/or lone working
* Experience of working in rehabilitation services
* Demonstrates ability to use initiative to deal with situations as they occur
* Ability to work collaboratively with the multidisciplinary team
* Experience of providing supervision
* Knowledge of the Recovery Model and Social Inclusion
* Knowledge of rehabilitation principles and how to apply in practice
* Ability to work creatively and compassionately to support service users and to develop collaborative goals
* Ability to motivate, engage and support service users in their own recovery and to achieve their goals.
* Excellent communication skills, both verbally and in written form
* Ability to develop rapport and therapeutic relationships with service users
* Demonstrates effective interpersonal skills
* Maintains professional boundaries
* Ability to work autonomously and as part of a team
