We are looking for a passionate and qualified Level 1 Social Worker to join our Adult Social Care team within the Newcastle District. We are a community-based team within the older persons/physical disability team, working with adults over 18 years of age with a range of physical and mental health needs.
We are a supportive and dynamic organization that provides opportunities to progress and develop within your social work career. We are delighted to offer a specialist Social Work Learning Academy (SWLA), an internal academy providing ongoing social work training and professional development. On appointment, you will be enrolled in the SWLA and aligned with the appropriate British Association of Social Work Professional Capabilities Framework. Your SWLA 'stage' (Stage 1 - ASYE, Stage 2 - Social Worker, or Stage 3 - Advanced Social Worker) will be determined based on your prior experience and CPD.
The post holder will be responsible for their own CPD and will play a key role in supporting personalisation and social inclusion agendas, ensuring quality public-facing services including health and social care. Responsibilities include:
1. Carrying out assessments, including holistic 'needs'-led assessments, care and support planning, reviews, and initiating multidisciplinary assessments where appropriate.
2. Being an integrated and supportive team member, planning, actioning, monitoring, and evaluating work under supervision.
3. Collaborating with other Trust services to provide seamless care for adults and carers, focusing on safe and effective transitions.
4. Participating in duty/ACCESS or priority rotas, conducting assessments for health and social care services using various assessment tools, including a Risk Screening Tool.
If you are looking for a new challenge, enjoy variety in your work, and wish to be part of a team striving to make a difference by supporting people to live independently, we want to hear from you.
