Clinical Coordinator – Wirral Community Health and Care NHS Foundation Trust
Join us in delivering high‑quality care for children and young people with Special Educational Needs and Disabilities (SEND). This role supports collaborative, integrated planning and improves multi‑agency coordination across health pathways.
Key Responsibilities
* Coordinate clinical input for SEND pathways – Ensure timely, high‑quality clinical contributions to Education, Health and Care Plans (EHCPs) in line with statutory requirements.
* Facilitate multi‑agency collaboration – Act as liaison between health services, education, and social care to support integrated planning and delivery.
* Support SEND inspection improvement actions – Deliver on specific improvement priorities identified through SEND inspections, ensuring compliance and transformation.
* Provide oversight & guidance – Offer expert advice to frontline teams, helping manage complex cases and reduce variation in care.
* Monitor performance and quality – Track service delivery against key performance indicators, contributing to audit, evaluation, and continuous improvement.
* Engage with families & professionals – Support effective communication with families, carers, and professionals to ensure coordinated care and positive outcomes.
Essential Qualifications and Experience
* Registered Nurse or registered allied health professional, or registered social worker (Education or Early Years practitioner also accepted).
* Degree level qualification in early years, education, nursing, AHP (speech and language therapist, physiotherapist, occupational therapist), or social work.
* Experience working with children, young people (0‑25 years) and their families.
* Evidence of continuing professional development in child and SEND care, with demonstrated ability to build positive relationships in a complex system.
* Postgraduate qualifications (master’s or diploma) in health or leadership, evidence of navigating systems change and transformation.
Essential Knowledge & Skills
* Review case notes, summarise clinical needs and support EHCP assessment and reviews.
* Self‑directed learning and reflective practice.
* Sound understanding of Clinical Governance.
* Knowledge of challenges facing children and young people with SEND.
* Experience delivering training and managing safeguarding issues.
* Strong communication, influence, negotiation and problem‑solving skills.
* Analytical, prioritising and organisational skills; strong IT competency.

* Flexibility to travel to different sites and adapt to changing demands.
* Commitment to improving the SEND system.
* Empathy, compassion and ability to build positive relationships with stakeholders.
* High‑standard focus, time‑management and analytical ability.
Pay & Contract
Band 6 – £38,682 to £46,580 a year (pro rate). Permanent contract, full‑time.
