The role will include tasks such as:
1. Helping people leave hospital promptly and transition safely back to independent living at home.
2. Visiting Service Users in their homes to provide care and support as outlined in the Care Plan.
3. Providing personal care, administering medication, monitoring vital signs, assisting with exercises, and domestic tasks if needed.
4. Liaising with medical professionals such as district nurses and pharmacies.
The goal is to rehabilitate, maximize potential, and support Service Users in activities that promote independence and monitor well-being.
Working hours are between 7am-10pm, with shift patterns of 7am-2pm and 4pm-10pm, including alternate weekends.
We offer competitive pay rates of £13.43 per hour on weekdays and £15.50 per hour on weekends.
*Note: The hourly rate is subject to holding an NVQ Level 2 or above in Health and Social Care.
