Responsibilities
* Ensure safe and smooth discharge of patients from the hospital.
* Collaborate with patients, families, and healthcare professionals to coordinate post-discharge services.
* Develop and implement appropriate care plans tailored to individual patient needs.
* Act as a liaison between hospital staff and external care providers to ensure continuity of care.
* Address any challenges or concerns to facilitate a seamless transition for patients.

Requirements
