Salary: From £36,000 p.a. FTE (pro-rated for part-time) dependent on skills, qualifications and experience

Hours: Part time (0.6 FTE), working full days on Wednesday, Thursday & Friday
About The Role
You will be part of our growing team of Practice clinical pharmacists and pharmacy technicians. You will develop through team clinical supervision sessions and benefit from direct mentoring with an experienced GP/ Lead Pharmacist.
We welcome applications from those who are:
* experienced in primary care, or
* those wishing to move into the primary care sphere, where you will have the opportunity to access and complete NHS England accredited training pathway that equips you to practice and prescribe safely and effectively in a primary care setting (CPPE).
Main duties
To work as part of our practice clinical pharmacy team, led by our senior clinical pharmacist, to:
* Improve patient safety, health outcomes and efficiency by providing direct, accessible and timely Clinical Medicines expertise by working as part of a multidisciplinary practice team.
* Support proactive transfer of medicines optimisation workload from GPs and ANPs to the in-house clinical pharmacy team
* Promote safe, evidence based, cost effective prescribing in line with ICB and NHS requirements
* Provide expert pharmaceutical and medicines optimisation advice to practice staff and patients
* Provide prescribing support in areas such as interim and annual telephone and face to face medication reviews and follow ups, including review of medication-related monitoring results
* Medicines reconciliations (hospital outpatient and discharge letters; new patient registrations; amber drug requests)
* Implementation of prescribing guidelines
* Medication monitoring and safety
* Quality improvement initiatives in prescribing and medicines usage
* Support medicines-related training and education within the practice
This role is pivotal to improving the quality of care and operational efficiencies, so it requires good attention to detail and practice procedures, as well as motivation and passion to deliver an excellent service within a General Practice environment.

Primary Duties And Areas Of Responsibility
* Patient facing long-term condition and medication review clinics: See patients with single or multiple medical problems where medicine optimisation is required; review ongoing medicine plans and provide recommendations to clinicians for medicine improvement. Medicines support and advice in person or by telephone. Provide patient facing clinics or telephone support for questions about medicines in the practice.
* Medicine information to practice staff and patients: Respond to medicine-related enquiries, suggest solutions, provide follow up to monitor effects.
* Unplanned hospital admissions: Provide targeted support for at-risk patients, manage medicines at discharge, reconcile medicines post-discharge, and liaise with community pharmacists.
* Signposting: Ensure timely referrals to appropriate healthcare professionals.
* Repeat prescribing: Support safe and timely repeat prescribing, electronic prescribing and monitoring.
* Risk Stratification: Identify cohorts at high risk of harm from medicines via practice computer searches.
* Service Development: Contribute pharmaceutical advice for new services and develop specialist areas of interest.
* Information Management: Analyse and present medicines data to highlight issues and risks.
* Medicines Quality Improvement: Undertake audits and implement changes with the practice team.
* Medicines Safety: Implement changes from MHRA alerts and other guidance; support formulary recommendations and related procedures.
* Cost Effectiveness: Support quality and cost-efficiency initiatives.
* Education And Training: Provide education to the primary healthcare team on therapeutics and medicines optimisation.
* Care Quality Commission: Ensure the practice is compliant with CQC standards where medicines are involved.
* Public health: Support vaccination campaigns and provide specialist knowledge on public health programmes.
Person Specification
Qualifications - Essential: Must be registered as a pharmacist with the General Pharmaceutical Council; CPPE Primary Care Pharmacy Education Programme exemption or demonstration; independent prescribing qualification (held, working towards or willing to complete); understands relevant standards of practice; adheres to performance policies.
* Desirable: Membership of Primary Care Pharmacy Association (PCPA); Membership of the Royal Pharmaceutical Society
Personal Attributes - Essential: Strong communication and collaboration; ability to manage risk; accountable for expertise and service; able to plan and prioritise; attention to detail; strong written and spoken communication; flexible and team-oriented; knowledge of governance, safeguarding, information governance, health and safety; ability to explain complex information clearly; understands wider determinants of health; able to work independently and in a team; understanding of rational prescribing.
* Essential Experience: Broad knowledge of common acute and long-term conditions in general practice; ability to integrate with community and hospital pharmacy teams; evidence-based healthcare principles.
* Desirable Experience: Experience in primary care or GP practice
* Other: Current Enhanced DBS; safeguarding and mandatory training
