Overview
We are looking for an experienced Occupational Therapist to lead the East Ross Discharge to Assess team. The post holder will provide assessment and specialist interventions to adults in hospital, or recently discharged from hospital, in order to facilitate an early and supported transition from hospital to home. The team will work to improve the patient journey, from the initial point of a hospital stay, preventing delays through early and effective planning and ongoing assessment at home. Multidisciplinary and multi-agency working is an essential component of the post, and there may be a need to work outside normal office hours, therefore some flexibility is required.
Responsibilities
* Lead the East Ross Discharge to Assess team; provide assessment and specialist interventions to adults in hospital or recently discharged to facilitate transition from hospital to home.
* Improve the patient journey by early planning and ongoing home-based assessment; develop rehabilitation plans for support staff to follow.
* Work with patients in their own homes and collaborate with multidisciplinary and multi-agency partners; supervise other staff as required; contribute to continuing professional development.
Qualifications and Requirements
* You must be registered as an Occupational Therapist with the Health and Care Professions Council (HCPC).
* Experience in the Discharge to Assess philosophy; experience in team working and effective communication skills.
* Experience supervising staff and a commitment to continued professional development are essential.
* A driving licence and use of own car for work purposes is essential.
