A focus of the role is safeguarding vulnerable people in line with ECC statutory obligations.

* Proven track record of building working relationships with partners and external agencies.
* A certificate in First Aid and/or knowledge of Sign Language is desirable.
Accountabilities
* Assess social care needs and provide support to vulnerable adults to enable healthy and independent living, upholding professional standards and contributing to service improvement in line with four key principles.
* Contribute to the development of care and support plans that gain commitment to solutions meeting each individual’s needs.
* Collaborate with the placement team to support individuals in commissioning care from appropriate providers; develop Direct Payments where applicable.
* Work within statutory legal frameworks and organisational guidelines (e.g., Care Act 2015, MCA/DoLs/CHC) and determine when statutory intervention is required.
* Highlight safeguarding issues and implement plans to protect vulnerable adults.
* Build strong relationships with clients, families and carers; provide practical advice, support, and advocacy when needed; enable independence and health.
* Collaborate with internal and external professional partner agencies to deliver joint objectives and improve outcomes.
* Provide information and advice on accessing third-party agencies (e.g., benefits, housing, immigration).
* Attend meetings, reviews, case conferences and court hearings as required; provide information, insight and evidence and prepare reports when appropriate.
* Work with annual performance targets within the performance management framework.
