We’re looking for a caring, proactive and skilled Assessment and Review Officerto join our Home First team. About the Role
As part of our Reablement Service, you’ll:
1. Complete timely assessments, reviews and support plans that reflect each person’s needs, goals and strengths.
2. Coach and empower people to take an active role in their own recovery, using a reablement approach that builds confidence and independence.
3. Assess and manage risk, ensuring safe, person-centred care.
4. Work closely with colleagues, families, carers and partner agencies to deliver consistent and effective support.
5. Support the deployment of care staff and contribute to the day-to-day delivery of a high-quality, CQC-compliant service.
6. Teach and demonstrate the use of mobility aids and assistive technology.
7. Maintain accurate, up-to-date electronic records and documentation.
This is a varied and rewarding role where no two days are the same. You’ll make a real and lasting difference to people’s lives every day.

8. Experience of assessing adult care needs, risk management and care planning.
9. A background in social care, health care or community settings.

11. The ability to work flexibly — independently and as part of a team — to meet service needs 7 days a week.
12. A full driving licence and access to your own vehicle.
You’ll also be self-motivated, compassionate, and confident in supporting vulnerable adults while maintaining the highest standards of safeguarding and professionalism.
Qualifications:
13. Diploma/NVQ Level 3 in Health or Social Care (or willingness to work towards).
14. Competency certificates in care, manual handling, and simple aids/adaptations (or willingness to obtain).
