Job overview Combined Community and Acute Care group are looking to recruit motivated and enthusiastic Nurse Advisors to work on on a part time or full time basis. This role is varied ensuring robust procedures are in place to deliver high standards of clinical care and safety. To be involved in the delivery of current and new projects city-wide, offering appropriate timely intervention to support recovery and independence. This role is fundamental offering a wide choice of referral routes to prevent avoidable hospital admissions, reduce duplication for service users ensuring value for money.
