Job location: Dawley Medical Practice, King Street, Dawley, Telford, Shropshire, TF4 2AA. Working across 3 practices with potential future changes.


Job Description

The post holder will join a high performing team of clinical pharmacists and a pharmacy technician within Wrekin PCN, serving a patient population of around 50,000 patients. The role involves working within clinical and professional boundaries in a patient-facing capacity as part of a multidisciplinary team to manage medicines on transfer of care and to implement systems for safer prescribing. Structured medication reviews will be performed for patients with polypharmacy, especially for those with frailty or multiple comorbidities, as well as reviews for specific long-term conditions within competency.

Key duties include working with stakeholder organisations to improve safety and quality of care, managing medicines on transfer of care, delivering medicines optimisation initiatives in GP practices, providing specialist professional pharmaceutical advice and services, and contributing to local and national formulary guidelines. The role supports quality improvement, clinical audit, medicines safety, antibiotic stewardship, and may contribute toward practice financial stability through medicines optimisation and QOF targets.

Compliance with organisational policies and procedures is required. The post holder may be required to work across three practices now and in the future.

* Medicines reconciliation on discharge from hospital
* Medicines optimisation
* Repeat prescribing and promoting Electronic Repeat Dispensing
* Monitoring of High Risk Drug Alerts in accordance with local policies and procedures
* Medicines safety and quality improvement
* Implementation of local and national formulary guidelines
* Education and training


Person Specification


Experience

Essential

* An appreciation of the organisational nature of PCNs, GPs and general practices.
* An appreciation of primary care prescribing, rational prescribing concepts, and improvement strategies.
* Excellent interpersonal, influencing and negotiating skills.
* Excellent written and verbal communication skills; ability to explain complex information clearly.
* Good IT skills (Microsoft Office Word and Excel).
* Ability to obtain and analyse complex technical information.
* Ability to prioritise, problem-solve, and refer to seniors or GPs when appropriate.
* Ability to work under pressure and meet deadlines.
* Ability to gain acceptance for recommendations and motivate compliance.
* Ability to work effectively independently and as a team member.
* Accountability for delivering professional expertise and direct service provision.

Desirable

* In-depth therapeutic and clinical knowledge with evidence-based healthcare principles.
* Ability to plan, manage, monitor, advise and review medicine optimisation issues for long-term conditions.
* Experience with EMIS Web clinical system.


Qualifications

Essential

* Masters Degree in Pharmacy (MPharm) or equivalent
* Mandatory registration with General Pharmaceutical Council
* CPPE Primary Care Pathway training or working towards/completion

Desirable

* Membership of the Royal Pharmaceutical Society
* Independent Prescribing qualification
* Postgraduate diploma level or equivalent training/experience


Other

* Self-motivation
* Adaptable
* Full driving licence and use of own car
* Willingness to travel between practice sites
* In-date DBS check
* Immunisation status
* UK work permit if required

Desirable

* Safeguarding adult and children level three
* Up-to-date mandatory training applicable to the role, including Basic Life Support
