Main duties of the job
Support the pharmacyadmissions team in identifying patients with pharmaceutical problems andprioritising appropriately to quickly resolve problems.
Act as supervisor andmentor for rotational pharmacists and MMTs and provide educational support fornew members of staff requiring medicines reconciliation training.
Participate inclinical pharmacy services including training, prescription chart review,patient counselling, adverse drug reaction reporting and therapeutic drugmonitoring and individualisation of patients drug therapy.
optimise medicinesmanagement within the Trust and the Pharmaceutical Care of those patients,ensuring that drug therapy is provided in a safe, effective and cost-effectivemanner tailored to the clinical need of individual patients.
Be available forexpert consultation with medical and nursing staff on matters relating to safe,economic and effective drug usage, including implementation of InfectionControl policies, Medicines Management Policy and the Trust Formulary.

Person Specification
Experience
* NHS experience is Highly Desirable including Clinical Pharmacy experience.
Qualifications
* Professional Registration with the General Pharmaceutical Council (GPhC).
* Masters Degree in Pharmacy or UK recognised equivalent.
* Post Graduate Diploma in Clinical Pharmacy.
