Job Overview

A rare and exciting opportunity has become available for a Band 6 Occupational Therapist to join our community service within Anglesey. You will work as part of an MDT of Occupational Therapists, Physiotherapists and support therapists. As a team we work within a wider MDT including District Nurses, Specialist Nurses, and Social Workers. The post holders will be required to complete assessments, interventions and rehabilitation programmes to aid the recovery, improve function, reduce the risk of de-conditioning and improve the health and functional ability of individuals within their own homes. The varied case load includes short term Rehabilitation, Orthopaedic pathway, Discharge to Recover and Assess, and Palliative Care. You will need a knowledge of a range of rehabilitation interventions, complex manual handling and knowledge of diverse conditions as well as a good understanding of current issues that affect community and hospital services.

Main duties of the job

As an autonomous practitioner you will:

Provide a flexible and comprehensive service to the community area you are responsible for
Confidently carry out comprehensive and professional interventions for patients with complex needs incorporating physical, personal and psychological needs in relation to self-care, leisure and productivity
Have excellent organisational and communication skills
Provide supervision, and development of support staff and students within your team
Maintain links with the immediate team as well as wider community team alongside external agencies to maintain and develop the Trusts Occupational Therapy service
Have the ability to use computer systems in order to update clinical information necessary for the job role including data collection and audits
Maintain and update own knowledge as required by the HCPC – active encouragement is promoted to seek internal and external training and development opportunities

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
