Job Summary
We’re looking for a Band 6 Older Adult Specialist to join our Older Adults Community Mental Health Team (CMHT) and Living Well Team in Ilkeston. This brand-new role offers the opportunity to shape meaningful support for older adults living with functional mental health conditions in the community.
Responsibilities
* Clinically responsible for supporting the delivery of a flexible, responsive local service and advancing an Integrated Wellbeing Pathway.
* Collaborate with colleagues, key partners and the local community to develop services that meet mental health needs.
* Use a wide range of skills, seeking advice and collaboration when required.
* Undertake comprehensive person‑centred psychosocial assessments, risk assessments and formulation of presenting mental health needs.
* Build therapeutic relationships, develop a care plan and empower individuals to take control over their health.
* Motivate individuals to improve their mental health by developing self‑care skills, building resilience and utilising community resources.
* Maintain relationships with statutory, VCSE and other community partners.
* Travel across the geographical area as required.
Qualifications & Experience
* Essential:
* Relevant qualification (Diploma in Nursing, Mental Health Nursing, Degree in Occupational Therapy).
* Post‑qualification experience and completed preceptorship or equivalent.
* Experience working with older adults with functional mental health diagnoses.
* Experience of community working.
* Good verbal/non‑verbal communication skills.
* Good IT skills.
* Able to work independently and as part of a team.
* Current UK professional registration.
* Desirable:
* Assessor qualification.
* Further relevant training.
* Experience linking with partner agencies.
* Understanding of the Living Well programme.
* Good time management.
