Main duties and responsibilities:

· Support the Joint Heads of Family Support and the Director of Wellbeing, Family Support and Integrated Governance by undertaking practical and administrative activities that contribute to and enhance the effective co-ordination of our family support services.

· Co-ordinate social media, including the production of short films for a range of social media platforms to explain the link between family support and well-being and engage potential donors in funding these services

· To support our Embedded Fundraisers to develop and maintain relationships enabling patients and families to take forward their interest in being a supporter

· Gather feedback from families and external partners about the impact of family support and how this service can develop and improve. Using mentimeters or appropriate evaluations for user feedback.

· Ensure that appropriate monitoring reports are provided as and when required to funders, other partnerships, senior management etc.

· Hold overall accountability for the programme of Family Support events and activities, working with team members to ensure that each event is bespoke to the needs of participants, with appropriate health, safety and safeguarding measures in place

· Act as Team Leader for the Carer Support Volunteers

· Work with individual service leads in the Family Support team and the communications and media team to help to develop secure self-referral to our carers services from our new website

· Support the Lead for Carers Services and the Social Work team linked to our integrated community team in helping to monitor support plans for family members to ensure positive outcomes are being met. Leading Carer groups and courses with volunteer support, supporting the Social Work team on IPU to offer practical and emotional support to the families in a timely way to ensure smooth discharge / transition of care and holistic care.

· Manage a small caseload of appropriate capacity providing direct practical, social, and emotional support including one-to-one work, ensuring a person-centered approach having respect for customs, religious beliefs and doctrines. Including supporting the Social Work team on IPU as needed.

· Be able to provide advice, advocacy, and support to ensure the views of adult family members are heard

· Ensure accurate and timely records are maintained on our electronic patient information system

· Maintain excellent working relationships with colleagues and partners internally and externally

· Bring to the attention of your line manager, or in their absence, the Chief Executive, any situation of concern or safeguarding and/or all matters that may compromise the responsibility and/or reputation of the Hospice.

· Recognise and avoid and mitigate situations that may be detrimental to the health and wellbeing of individuals.

· Act as an ambassador for the Hospice at all times

Key Accountabilities, Responsibilities & Tasks

Role Specifics

· Pro-active approach to work and problem solving, and the ability to spot and deal with issues as they occur

· Excellent organisation, prioritisation, project co-ordination skills.

· Proven listening skills, with the ability and skill to lead and influence others

· Advanced understanding of confidentiality, consent, capacity, data protection and the general data protection regulations

· Able to deliver on deadlines and sudden pressures whilst maintaining accuracy and attention to detail

· Numerate and literate, able to draft documents and compile reports

· Able to work under own initiative, under pressure within structured timescales.

Qualifications, Skills, Experience, Knowledge & Approach

· Excellent general education

· Educated to degree level (desirable)

· Professionally or vocationally qualified in therapy and/or rehabilitation with at least 3 years in practice including experience as an autonomous practitioner managing a caseload.

· Excellent attention to detail, able to proof read documents would be an advantage

· Experience of coordinating events for families or family carers

· Experience or working alongside volunteers and (desirable) working as a volunteer team leader

· Experience of service co-ordination and development

· Experience of palliative care (desirable)

· Able to speak confidently in public at events and in representing the organisation

· Works effectively within a team, generates new ideas and working practices without being prompted.

· Experience of using electronic patient information systems (or equivalent) Microsoft office, secure email at base and remotely

· Line management experience (desirable)

· Excellent communication skills with the mental agility to 'think on feet' and deliver safe practical solutions

· Resilience and compassion when faced with difficult, challenging people and complex situations

· Self-motivated and able to work on own initiative within a team environment

· Excellent communication skills, with the ability to assimilate multiple sources of often conflicting information, appropriately deliver sensitive and difficult news and facilitate difficult decisions

· See the potential in others and understands the impact of their actions on colleagues

· To have a visible and approachable presence

· Can communicate in more than one language relevant to the population served (desirable)

· Members of the general public as carers and family members.

· Colleagues and managers in your immediate team/organisation and across specialties and organisations

· Actively participate in local, national and international networking to maintain and develop expert level of knowledge and to act as a resource for education and clinical expertise

· The post holder is expected to take significant responsibility and autonomy for their area of work and work with a range of people at all levels internally and externally

· Act in accordance with the Code of Professional Conduct and Scope of Professional Practice and be accountable for his/her actions at all times

Mental and Physical Consideration. Working Conditions & Environment

· The post holder will be required to work in a high volume, fast paced environment. Frequent interruptions should be expected. Competing priorities are common place.

· The post holder will come into contact with emotional circumstances, through speaking to patients, families staff and volunteers and/or information received in the course of this role.

· The post holder will be required to have the physical capacity to be able to deliver therapies and activities

· Required to sit at a computer.

· The post holder may be required to visit various locations and will need to have the availability to travel in a timely and effective way

Health & Safety

· Evidence of a positive attendance record

· Equally able to work on own initiate and as part of a collaborative team

· Understand and comply with all Infection Prevention and Control Health and Safety, Fire regulations

· Ensure own safety and that of others in the course of work

· Report any accidents or incidents

Safeguarding

· Act in a manner at all times to safeguard the interests of individual patients/clients and their families and justify public trust and confidence in the Hospice of St Francis

Job Types: Part-time, Temporary

Contract length: 12 months

Pay: £27,000.00-£35,744.51 per year

Expected hours: 21.5 per week
