The Patient Safety Team manages all aspects of incident reporting, incident responses, incident notification and supports the Trust as a

Learning Organisation. This includes use of the incident software, monitoring progress with incident responses, escalation of concerns regarding patient safety, liaising closely with the patient safety representatives within the clinical divisions, provide training and reports on themes, trends and outcome from incident reporting.

The Patient Safety Team (PST) has responsibility for managing all aspects of incident reporting and responses under the Patient Safety Incident Response Framework.

The PST supports the Trust to be an exceptional Learning Organisation. Our work includes:

Managing and optimising the use of the incident reporting system.

Monitoring progress with incident reviews and responses.

Escalating concerns relating to patient safety issues to the appropriate teams and senior managers.

Working closely with patient safety representatives across the clinical divisions.

Providing training and assistance with incident responses

Producing reports on themes, trends, and outcomes from incidents. Using data to promote learning and improvement

Includes but not limited to:

To assist the Associate Director of Quality and the Patient Safety Lead, to promote national patient safety initiatives throughout the Trust. To act as a source of expert advice and support to Divisions on patient safety matters

To liaise with other Trust level risk management advisors (e.g.: Claims Manager, Head of Health & Safety and Risk, Manual Handling advisor, PALS and Complaints team, Infection Control Team, Tissue Viability Nurse, Resuscitation Officer, Quality Assurance team) to ensure the identification of patient safety issues and implementation of actions to reduce harm to patients across the Trust.

In liaison with the Patient Safety Lead and other key staff, develop and implement training programmes in patient safety issues in line with the Patient Safety Incident Response Framework.

To work collaboratively with other members of the Patient Safety Team to ensure that robust systems are in place throughout the Trust to enable the identification of risk through the reporting and investigation of incidents

To liaise and work alongside Divisions about their systematic approach to monitoring progress on action plans

To advise Divisions on adding identified local risks to their local risk register and issues log and escalation where appropriate to other Trust level risk registers (RR)/ issues logs (IL) such as Patient Safety RR / IL, Medical Devices RR / IL.

To liaise with the Concerns & Complaints Manager, PALS and Legal teams to identify risks arising out of complaints and claims, ensuring that these are reported to the Patient Safety Committee for action and ongoing monitoring.

In collaboration with key staff, to develop patient safety related policies and procedures for approval and ratification by appropriate committees

To use as appropriate and advise Divisions on their use, the various tools

to assist in the review and assessment of patient safety matters such as the SBAR tool, Manchester Patient Safety Framework (MaPSaF), and methodologies within PSIRF

To monitor progress on the submission of incident responses such as Patient Safety Incident Investigations (PSII) or Patient Safety Reviews (PSR)

To assist in the co-ordination of the Trusts review panel for incident responses such as PSII's and PSR's.

To engage in the decision making process with Trust Executive on the appropriateness of incident responses and proportionate responses.

To maintain the Trust's Patient Safety intranet to ensure that procedural documents are kept up to date and are widely available

To undertake projects identified by the Associate Director of Quality and the Patient Safety Lead

To monitor and collate information from designated leads in order to compile reports for Trust level committees.

Promote and aid Divisions with their compliance with the Trust's obligations under the statutory Duty of Candour.
