Overview
Mosaic PrimaryCare Network consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Bramblys Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients. We are looking for an experienced paramedic to join us at Odiham and Old Basing Health Centres, ideally with a prescribing qualification.
The health centres are semi rural and are located in a small town and village, with staff working across both sites.

Overview of duties: To provide a high-quality, cost-effective service to the patients of the PCN; diagnosing and, if qualified, prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.
You will be working as part of a clinical team with GPs, Practice Nurses and supporting Healthcare Assistants. Your primary task will be to work within the duty team, assessing and treating same-day acute presentations. These will typically be patients presenting with a wide range of minor illness, urgent and same-day requests for treatment. There is scope for chronic disease management in addition to same-day care, depending on experience. You will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
You will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. You will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner.
You may be asked to do home visits depending on need but the practice has a low home visit rate and is supported by a home visiting service, so the need will be minimal. You would not be asked to do palliative or end-of-life visits. Individual practices may wish to invest in training to enable the job holder to support the team in chronic disease management.
Clinical practice and responsibilities
* Act as an autonomous clinician with the assistance of the clinical team
* Work as part of a MDT within the PCN
* Assess and triage patients, including same-day triage and, as appropriate, provide definitive treatment (including prescribing medications following policy, PGD, NICE and local clinical guidelines and local care pathways, or make necessary referrals to other members of the primary care team)
* Advise patients on general healthcare and promote self-management where appropriate, including signposting to the PCN Social Prescribing service and other community or voluntary services
* Perform and interpret ECGs, undertake investigatory procedures as required, and collect pathological specimens (blood, swabs and other samples) within the scope of practice
* Support delivery of anticipatory care plans and lead certain community services
* Provide an alternative model to urgent and same-day GP home visits for the network and clinical audits
* Communicate effectively across organisations to deliver a person-centric service
* Communicate pro-actively with all colleagues across an MDT and with patients and carers as applicable
* Ensure clinical practice is safe and within competence
* Clinically examine and assess patient needs and plan care accordingly
* Take a comprehensive patient history, perform physical examinations and use clinical judgement to identify potential diagnoses
* Refer for investigations and/or appropriate specialists when required
* Independently decide on and carry out treatment, including prescribing of medicines if qualified
* Plan and provide skilled, competent care to meet health and social care needs
* Ensure continuity of care including follow-up consultations/visits
* Assess and evaluate, with patients, the effectiveness of treatment and care, making changes as needed
* Support patients to adopt health promotion strategies and self-care
Administration and professional responsibilities
* Participate in the administrative and professional responsibilities of the practices within the PCN
* Ensure accurate and legible notes of all consultations and treatments are recorded in the patient’s computer record
* Ensure accurate completion of all necessary documentation associated with patient care
* Ensure collection and maintenance of statistical information required for regular and ad-hoc reports and audits
* Attend and participate in clinical meetings
* Recognise and work within own competence and professional code of conduct
* Prioritise, organise and manage own workload to maintain quality
* Deliver care according to NSF, NICE guidelines and evidence-based care
* Assess effectiveness of care delivery through self and peer review, benchmarking, audit and formal evaluation
* Participate in quality governance systems and processes
* Support and participate in shared learning across the practice
* Understand and apply legal issues that support the identification of vulnerable and abused children and adults; be aware of statutory requirements
* Alert other team members to issues of quality and risk
* Assess own performance and take accountability for own actions
* Contribute to team effectiveness by reflecting on activities and suggesting improvements
* Work effectively with individuals in other agencies to meet patients’ needs
* Manage time, workload and resources effectively; be punctual and professional
Communication
* Utilise sensitive communication styles to ensure patients are fully informed and consent to treatment
* Communicate effectively with patients and carers, considering different levels of understanding, cultural backgrounds and communication preferences
* Anticipate and address barriers to communication
* Maintain effective communication within the practice and with external stakeholders
Training and personal development
* Training requirements monitored by yearly appraisal in accordance with PCN requirements
* Personal development encouraged and supported by the PCN; stay up to date with developments
* Maintain continued education by attendance at courses and study days for professional development
* Develop and maintain a Personal Learning Plan
Qualifications and person specification
* Educated to degree/diploma level in Paramedicine or equivalent experience
* Registered with the Health and Care Professions Council (HCPC)
* Completed two-year Consolidation of Learning period as a newly qualified paramedic
* Further three years experience as a band 6 (or equivalent) paramedic
* Working towards masters level or equivalent capability in paramedic practice; within six months of reimbursement, completed and signed off within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice
* Experience working within Primary Care/GP Practices
* Complete masters level
Driving
* Must have a full valid driving licence and access to your own vehicle
