Job Summary
An exciting opportunity has arisen for an enthusiastic and proactive Pharmacy Technician to join our Primary Care Network (PCN) Care Home Team. The postholder will work across multiple GP practices and care homes within Acclaim PCN to support safe, effective, and person-centred medicines use for care home residents.
You will work closely with PCN Clinical Pharmacists, GPs, care home staff, and wider community teams to ensure medicines optimisation, improved health outcomes, and reduction in medication-related harm for this vulnerable population.
This role is part of the Additional Roles Reimbursement Scheme (ARRS) and supports the delivery of the Network DES.
Main Duties of the Job
* Contribute to structured medication reviews and medicines reconciliation, especially post-discharge.
* Support the safe, effective, and evidence-based use of medicines in care homes.
* Conduct audits and support quality improvement initiatives.
* Provide advice and training to care home staff regarding safe medicines handling, storage, and administration.
* Work closely with community pharmacies and wider MDT to improve continuity of care.
* Assist in the review of repeat prescribing processes and reduce medication waste.
About You
Essential:
* Registered as a Pharmacy Technician with the General Pharmaceutical Council (GPhC).
* Relevant experience in a primary care, community pharmacy, hospital, or care home setting.
* Good knowledge of medicines management and clinical governance principles.
* Strong organisational, communication, and interpersonal skills.
* Able to work both independently and collaboratively within a multidisciplinary team.
* Completion or willingness to complete the CPPE course which is a mandatory requirement for this PCN, ARRS funded role.
Desirable:
* Experience working in care homes or with older people.
* Knowledge of SystmOne.
* Understanding of the role of pharmacy technicians in primary care and the PCN DES.
