Position
HCP 0–19 Public Health Practitioner at Cambridgeshire Community Services NHS Trust.
Overview
Provide clinically effective, high‑quality public health services to children, young people and families under the direction of the HCP Team Leader within the 0–19 Healthy Child Programme across Cambridgeshire and Peterborough. Work within a corporate caseload with other team members to deliver family‑centred public health care and support the delivery of the 4, 5, 6 model of care.
Responsibilities
* Deliver universal services and address complex needs within the 0–19 Healthy Child Programme.
* Support the delivery of the 4, 5, 6 model of care and work with the SCPHN to improve outcomes in the six high‑impact areas.
* Collaborate in partnership with key agencies such as Children’s Services and Education to address health inequalities and improve outcomes for families.
* Provide family‑centred public health services and assist in the provision of a high‑quality service to children and young people.
* Participate in the corporate caseload alongside other team members to support the HCP Team Leader.
Qualifications & Experience
* Current NMC registration and relevant experience.
* Fully qualified and experienced Registered Nurse or newly qualified Nurse.
* Will undertake Specialist Community Public Health Nursing qualification within 2 years of appointment.
Working Conditions
* Full‑time and part‑time vacancies available in both North and South localities.
* Work within a corporate caseload with other team members.
