The Role
We are currently seeking a passionate and dedicated Grade 1 Social Worker to join our acute care team in a busy, large private hospital.
As a Social Worker at Cabrini, you will:
12. Deliver high-quality patient care using sound clinical judgment and communication
13. Work collaboratively in a multidisciplinary team, including doctors, nurses, physiotherapists, occupational therapists, dietitians, speech pathologists, and external service providers
14. Conduct comprehensive psychosocial assessments
15. Develop and implement targeted intervention plans in partnership with patients, families, and carers
16. Contribute to safe and effective discharge planning and community referrals
What We’re Looking For
17. Strong communication, computer literacy, and documentation skills
18. Eligibility for membership with the Australian Association of Social Workers (AASW) – essential
19. Capacity to work within a performance-review and feedback framework – essential
20. Previous experience in a hospital setting – desirable
21. Solid knowledge and application of evidence-based practices in social work, including in mental health settings
22. A commitment to ongoing professional development and participation in quality improvement or research projects
