The Lead Care Manager functions as the leader for a group of Care Managers during the shift. The role involves ensuring quality care and services are provided and documented in alignment with residents' Service Plans (ISP) and Sunrise policies. The Lead Care Manager leads by example and may work variable hours, including some weekends and holidays.
RESPONSIBILITIES & QUALIFICATIONS
Essential Responsibilities:
1. Provide supervisory support, including managing attendance, call-offs, shift messages, and requests.
2. Support team members in providing Activities of Daily Living (ADL) care according to ISP, including bathing, grooming, hygiene, dressing, and escorting residents.
3. Encourage resident participation in life enrichment activities and ADLs.
4. Assist with meal service, coaching staff on hospitality standards.
5. Document care and services accurately, review documentation, and coach staff to bridge gaps.
6. Ensure compliance with timekeeping and break policies, monitor schedules, and coach staff as needed.
7. Work flexible hours, including holidays, evenings, and weekends.
8. Prepare resident group assignments and participate in crossover meetings with other Lead Care Managers.
9. Handle overnight resident groups and fill in for unfilled shifts.
Other Responsibilities:
1. Cross-trained and credentialed to administer medications if needed.
2. Lead by example in clocking in/out and taking breaks.
3. Support onboarding and training of new team members.
4. Maintain a safe environment, report incidents, and support safety practices.
5. Communicate resident condition changes, assist with ISP updates, and support family communication.
6. Handle conflicts, contribute to performance reviews, and project a professional image.
Qualifications:
1. High School diploma or GED (preferred or required by state).
2. Effective communication skills with residents, families, and staff.
3. State-required certifications, including Medication Management Certificate.
4. Experience with seniors and leadership skills.
5. Ability to delegate, make decisions, remain calm, and handle multiple priorities.
6. Strong organizational, problem-solving, and computer skills.
7. Ability to drive Sunrise vehicle if applicable.
