This is an exciting development opportunity for a Band 6 Mental health Nurse, Social Worker, or Occupational Therapist to join the Bradford and Airedale Early Intervention in Psychosis Assessment team as a Care Coordinator/Care Navigator.

The Early Intervention hub is a dynamic high performing service based in Bradford. The successful applicant will join an experienced, established team of supportive specialist mental health professionals and will work closely within a MDT framework.

The role focuses on specialist assessment of individuals presenting with potential first episode psychosis and those at risk of developing psychosis (At Risk Mental States) across the Bradford region. The successful postholder will undertake care coordination responsibilities working closely with individuals referred, their carers and support networks, offering supportive interventions during the period of care.

BDCT EIP are committed to supporting staff with regular clinical and caseload management supervision and offer a culture of learning and ongoing progression to help you grow. In line with National EIP safer staffing practices, staff caseloads are protected to enable effective, responsive high quality work.

Main duties of the job

Working in partnership with service users, carers, child and adult mental health services, primary care and other agencies, coordinate, assess and plan and provide programmes of care aimed at optimising social recovery for individuals recovering from a first episode of psychosis or those at risk of psychosis.

Detailed job description and main responsibilities

Essential criteria

1. Professional qualification in mental health nursing, social work, occupational therapy
2. Degree level education or equivalent experience
3. Registration with Professional Body (e.g. NMC, SWE, BAOT)

5. Candidates with professional qualifications (such as RGN) with substantial experience of care coordination in a mental health service.
6. Experience of working with adults and/or young people and children with serious mental health problems

Desirable criteria

7. Experience of working in an Early Intervention Service/specialist psychosis service
8. Experience of working in a mental health team where assessment is the primary focus
9. Experience of working with people who use/ misuse substances
10. Experience of providing services across traditional boundaries.
