Community Staff Nurse Rapid Response

We have the equivalent of 3 full time posts available within the Rapid Response Nursing team covering the Amber Valley and Erewash localities.
We are a professional, friendly, and enthusiastic team who work closely with our integrated community colleagues to provide a holistic integrated service that enables and educates our patients to remain independent in their own homes, whenever possible, to prevent hospital admission and support a safe discharge home.
This role would be an ideal opportunity for either an experienced Band 5 Nurse seeking a change in direction or a newly registered nurse at the beginning of their professional career.
Main duties of the job
Provide high-quality nursing care to our patients in our local communities to keep them safe at home, prevent hospital admission and support hospital discharge home.
We have two bases across our two localities:
* Amber Valley – currently at Babington Hospital and in the spring relocating to our new purpose-built site in Belper.
* Erewash – at Long Eaton Health Centre.
There is an expectation to travel across the localities as and when required to meet the needs of our patient group. Amber Valley covers Ripley, Heanor, Alfreton and Belper localities whilst Erewash covers Ilkeston and Long Eaton and surrounding areas.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse and Senior Community Nurses in the management of the health care environment.
Implement / refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries/Team Up and local acute trusts.
As this is a rapid service, there will be a need to be able to adapt care to meet the patient demand which is ever changing.

Education / Qualifications
Essential
* Full NMC registered Nurse
* Evidence of recent relevant training

* Mentoring Qualification
Experience and Knowledge Required
