Overview
This opportunity is for a Band 6 Occupational Therapist to work within the Frailty Therapy team across Front Door Therapy Services, covering the Frailty Assessment Unit and areas aligned with the Emergency Care department. The team comprises Occupational Therapists, Physiotherapists, Therapy Assistant Practitioners and Discharge Managers who assess and plan therapy needs and discharge arrangements for patients admitted via the Accident and Emergency department. We are part of the wider Frailty MDT and work to ensure holistic, comprehensive assessments and timely treatment interventions in the most appropriate environment. Responsibilities
The post holder will provide comprehensive assessment and treatment for patients referred for Occupational therapy and work as an integral part of the multi-disciplinary team. They will contribute to delivering therapy and support comprehensive discharge planning, ensuring the patient’s wishes are at the forefront of decisions. Specific responsibilities include:
* Manage a daily clinical caseload of patients, including assessment, formulation and treatment; provide specialised therapy advice to MDT colleagues.
* Work closely with community partners to ensure optimal transfer of care.
* Deputise for the Team Leader on clinical and managerial issues within the team.
* Assist with training within the Department in support of new graduate Occupational Therapists, student therapists, therapy apprentices and support workers alongside other senior team members.

Requirements
Essential
* BSc Occupational Therapy
* HCPC Registration
Desirable
* Member of RCOT
Skills
* Highly developed clinical skills in an acute inpatient setting
* Excellent written and verbal communication
* Excellent organisational skills and time management
Experience
* 18 months post-graduate experience in relevant inpatient or general rotations
* Proven experience delivering high quality intervention for high-risk patients
* Experience in a fast-paced setting or Emergency Department
Knowledge
* Awareness of NHS and Primary Care developments including Clinical Governance, NICE and CQC
* Knowledge of discharge pathways and evidence-based practice
* Experience with complex assessments and discharge planning and outcome measures related to medical/frailty patients
