An exciting opportunity has arisen for a Band 4 Tissue Viability Assistant Practitioner to join our specialist Tissue Viability Nursing (TVN) service within Central Cheshire Integrated Care Partnership. This is a key clinical role supporting the delivery of high‑quality, evidence‑based wound care across community and acute settings. You will work closely with the Tissue Viability Specialist Nurses and wider multidisciplinary teams to support patients to achieve optimal wound healing, prevention of harm and improved quality of life. As a Tissue Viability Assistant Practitioner, you will: • Support the Tissue Viability Specialist Nurses in the assessment, planning, delivery and evaluation of person‑centred wound care • Deliver evidence‑based tissue viability interventions in line with Trust policies, clinical guidelines and best practice • Maintain accurate, timely and professional clinical documentation using systems such as EMIS and other relevant digital platforms • Support and deliver formal and informal education and training to staff within the community and hospital settings • Act as a role model for best practice in tissue viability, infection prevention and patient‑centred care • Contribute to service development, audit and quality improvement initiatives • Work collaboratively with colleagues across community nursing, acute services and the wider multidisciplinary team • Promote patient dignity, safety, safeguarding and professional standards at all times Mid Cheshire Hospitals NHS Foundation Trust provides comprehensive hospital and community services for East Cheshire (population 399K) and West Cheshire and Chester (population 357K).
