Overview
An exciting opportunity has arisen for a Band 7 Flow Lead in The Bay. The post is a full-time, permanent position covering the service over 5 days per week. The Flow Lead will support safe flow for patients requiring inpatient admission to a mental health ward and will work closely with inpatient wards and community teams to support admission and safe discharge following assessment and treatment.
The Flow Lead will have a relationship with the bed hub and support patients to be admitted as close to home as possible. They will also work with mental health services as well as third sector providers, so excellent communication skills are essential.
Responsibilities
* Provide leadership and act as a positive clinical role model through visible leadership to motivate and empower others.
* Work with a multi-disciplinary team (Mental Health Nurses; Support Workers; Social Workers; Occupational Therapist; Psychologist; Consultant Psychiatrist) to deliver specialist advisory resources for Bay Locality on matters pertaining to patient flow and capacity.
* Support patients from The Bay in wards across the footprint and in OAP beds.
* Collaborate with the bed hub, inpatient wards, community teams, mental health services, and third sector providers to ensure patients are admitted to and discharged from the most appropriate setting and at the most appropriate time.
* Contribute to locality and Trust-wide activities to improve patient flow across the mental health pathway from community to inpatient care.
