Job Description

Band 4: £27,485 - £30,162

Permanent contract

Full time hours: 37.5 per week

If so, this could be the opportunity for you. As a Cancer Pathway Co-Ordinator, you will play a vital role in supporting patients with a diagnosis of Prostate Cancer and those undergoing treatment for Head and Neck Cancer. Working under the guidance of the Lead Nurse and Clinical Nurse Specialists, you will help patients navigate the complexities of their treatment and follow-up care, ensuring they move smoothly onto their Risk Stratified Follow Up (SFU) pathway.

You will be instrumental in co-ordinating appointments, assessments and investigations, supporting holistic needs assessments, promoting self-management, and ensuring every patient experience reflects dignity, respect and the highest standards of safety. This is a rewarding role for someone who is organised, empathetic and committed to delivering personalised, patient centred care.

Key Responsibilities:

Proactively support and co-ordinate the needs of patients undergoing treatment for Head and Neck Cancer under the supervision of the clinical nurse specialist.

1. Co-ordinate the necessary assessments, investigations and appointments required prior to patients' starting treatment.
2. Co-ordinate the transfer of care between service teams, or facilitation of service provision across interfaces of care- for example primary and secondary care.
3. Refer and sign post to the clinical team as appropriate for decisions.
4. Monitor, audit and report response times to inbound telephone calls.
5. Contribute to the holistic needs assessment, care plan and review of patients in line with practitioner guidance, protocols and procedures.
6. Evaluate outcomes of care support with a Clinical Nurse Specialist.
7. Triage incoming calls from patients and initiate appropriate responses based on clear protocols and frameworks.
8. Make pre-planned outbound telephone calls to patients under the guidance of the Clinical Nurse Specialist.
9. Contribute to the clinical/quality management team reviews of any issues in relation to the patient pathway.

Co-ordinate access to patient information and education resources for patients.

1. Undertake a focused mapping of the range of information/education support resources available in the local health and social care sectors.
2. Support the delivery of information prescription delivery in consultation with health professionals and according to any agreed information pathways, including printing of information.
3. Collaborate and work closely with local providers of validated information resources, users and user groups, including the Macmillan Cancer Information and Support centre and other registered providers.
4. Contribute to the development of health and well-being events and information packs that support delivery of the recovery agenda, in particular, self-management.

Qualifications

Qualifications and Experience

Essential

* QCF or BTEC or equivalent (NVQ) Level 2 qualification in Health and Social Care/Clinical Health OR completion of NHSP Care Support Worker Development Programme with 3 months experience as a Health Care Support Worker
* English and Maths GCSE or Functional Skills Level 1
* Experience of working with people in a caring setting
* OR experience of working with people in a customer service setting
* Experience of data management
* Desire to deliver compassionate hands-on care in the hospital environment

Desirable

* QCF Level 3 (or equivalent) in care or modules relevant to the area.
* Level 2 English and Maths
* Experience within a hospital setting as a Health Care Support Worker

Skills and Knowledge

Essential

* Ability to work as part of a team
* Effective patient/client care skills
* Excellent verbal and written communication skills.
* Able to meet the minimum skill set within first twelve months of appointment with support and development where required (list of skills as defined in generic competency framework).

Desirable

* IT skills
* European Computer Driving Licence (ECDL)

Other

* Motivation to meet the patients needs for self and others.
* Ability to demonstrate confidentiality and trustworthiness.
* A willingness to be flexible and part of a team.
* Ability to juggle many priorities at one time, whilst remaining calm
