Are you an experienced, confident, and compassionate individual with a passion for community-based care? Are you ready to take the next step in your career? We are seeking a proactive Senior Support Worker to join our team and play a key role in delivering and shaping Carmarthenshire's Reablement, Discharge to Recover and Assess (D2RA), and Admission Avoidance services.

In this role, you will provide vital support to individuals, primarily older adults, helping them regain independence, build confidence, and continue living safely in their own homes.

This is a senior post within the Home First Intermediate Care Model, operated jointly by Carmarthenshire County Council (CCC) and Hywel Dda University Health Board (HDUHB).

In this senior role, you will not only deliver high-quality, person-centred care and support, but also provide guidance, mentorship, and day-to-day support to our dedicated team of Support Workers. You will help to coordinate care plans, monitor progress, and ensure that every individual receives the right support at the right time. Working as part of a supportive and forward-thinking team, you will be committed to making a real difference, one person at a time. You will both support, and be supported by, the Intermediate Care Leads to achieve the best possible outcomes and make a real difference.

The role includes:

* Supporting individuals with goal-setting and rehabilitation planning
* Working closely with service users, families, and professionals to promote independence and recovery
* Contributing to a collaborative, person-centred approach to care
* Triage and manage referrals in partnership with professionals from across the ICMDT, including GPs, community nurses, therapists, and social care.
* Work closely with ambulance services to support individuals at the point of crisis and help avoid unnecessary hospital admissions.
* Respond to falls-related referrals in collaboration with health professionals and develop appropriate short-term support packages to maximise recovery.
* You need to be flexible for the needs of the service and be able to respond and work county wide

Our rates of pay are highly competitive with similar posts across the Health and Social Care sector.
