We are looking for an experienced nursing lead who is passionate about improving patient experience, driving system-wide change, and enabling people to safely return home sooner. We're looking for a dynamic and motivated Clinical Lead Nurse to join our Pembrokeshire team, who will play a pivotal role in shaping safe, effective, and person-centred discharge pathways.

In this senior role, you will champion the Home First philosophy - ensuring patients receive timely assessment, coordinated discharge planning, and the right support. You'll provide daily operational leadership across acute, community, and intermediate care settings, working closely with nursing, therapy, social care, and third-sector partners to reduce length of stay and maximise time spent at home.

You will lead and develop several key teams, including Discharge Liaison, Porth Preseli Clinical Streaming Hub, Hospital @ Home, Acute Response, Home Support, and Community Falls services.

This role supports improved patient experience and outcomes by ensuring people can return home safely and promptly after a hospital admission. Guided by the "Home First" approach, the post holder will provide credible leadership across the wider Health and Social Care system to achieve this goal.

They will deliver daily operational coordination to ensure effective patient flow, focusing on timely assessment and discharge from acute, community and step-down settings to reduce length of stay, minimise readmissions and maximise time spent at home.

The post holder will line manage the Discharge Liaison, Porth Preseli Clinical Streaming Hub, Hospital @ Home, Acute Response, Home Support and Community Falls teams.

They will lead discharge planning, offering specialist knowledge on commissioning care and implementing local and national improvement initiatives. They will also drive pathway development to enhance flow.

Working autonomously, the post holder will apply highly developed specialist knowledge to support colleagues, promote professionalism and act as a model of integrity and leadership.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Hywel Dda University Health Board plans and provides NHS healthcare services for people living in Carmarthenshire, Ceredigion, Pembrokeshire, and bordering counties.

We have over 13,000 staff and together we provide primary, community, in-hospital, mental health and learning disabilities services.

We work in partnership with the three local authorities, as well as public, private and third sector colleagues, including our valued team of volunteers.

Our services are provided in:

Four main hospitals: Bronglais Hospital in Aberystwyth; Glangwili Hospital in Carmarthen; Prince Philip Hospital in Llanelli; and Withybush Hospital in Haverfordwest

Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire

Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings

47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers

Numerous mental health and learning disabilities services

They will also contribute to the strategic development of community services, ensuring alignment with Health Board and national objectives, and effectively communicate Pembrokeshire System priorities to staff within their remit.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
