We are looking to recruit a Support Worker to join our Targeted Care and Enablement Services Team.
Overview
The service is Occupational Therapist led, setting client-centered goals to regain as much independence as possible. This role involves activities of daily living in people’s homes.
Responsibilities
* Provide in‑home support to individuals to regain skills and independence following acute illness or injury, including activities of daily living such as personal care, mobility, meal preparation and medication management.
* Work on a rolling rota: 4 days on, 4 days off with shifts 7:00–13:00 and 16:00–20:00.
* Cover long‑term bridging while awaiting a package, expanding skills and knowledge.
* Manage workload via an electronic call monitoring system and adhere to the scheduled visits.
* Be prepared to travel within the County; wear uniform and use provided work phone and laptop; participate in ongoing training.
Requirements
* Full driving licence, car and ability to travel within the County.
* All Wales Induction Framework (AWIF) for Health and Social Care to be completed within the first six months of employment to support registration as a Social Care Worker with Social Care Wales within the mandatory timeline.
* Level 2 QCF in health and social care or equivalent.
* Basic conversational Welsh, with reasonable support provided on appointment to reach this level.
