Overview
Place Based Team - Warwick & Kenilworth works to provide patient centric care, encouraging prevention and self‑management. You will work closely with a multi‑disciplinary team, collaborating with GP’s and strengthening relationships.
Responsibilities
* Provide flexible and hardworking care, seeing patients in their own home environment.
* Operate under minimal supervision, within your competencies, and adhere to SWFT’s Lone Working Policy.
* Deliver high quality, caring and compassionate personal care and associated duties under the direction of registered staff, without direct supervision.
* Work collaboratively as part of the multidisciplinary team.
Qualifications
* At least one year of care experience, preferably in the community.
* Knowledge and skills in managing long‑term conditions, wound and pressure ulcer care, and end‑of‑life patients.
* Excellent communication skills and a strong team‑player attitude.
